REGISTRATION (PLEASE COMPLETE BY MARCH 15TH)

In order for you to be considered registered for trek, you will need to do four things.

1.  Complete the electronic registration at the link below.
Please read everything, and complete every field carefully, with your parents present.

2.  Complete and sign the parental permission and medical release form found at....Parental Permission Form and Medical Release   (Please note that adult leaders need this form also.)  

3.  Scan or take a picture of the signed parental permission form and email it to loslunastrek2019@gmail.com.

4. Please turn in your original, signed form to your ward YW President.  


After you do these things, you will be added to the list of trek participants registered.  Please check that list to be sure your name shows up. (It may take a few days as we will need to manually update the list.)
